Abstract | As doenças cardiovasculares ainda são a causa líder de mortalidade em todo mundo. A alta reatividade do sangue com qualquer superfície não endotelial é um desafio no desenvolvimento de Dispositivos de Assistência Ventricular de longa duração, geralmente fabricados em titânio grau II médico, que apesar de ser inerte, de baixa densidade e alta resistência mecânica não evita as respostas trombogênicas. A partir da modificação na superfície de titânio com Laser-Induced Periodic Surface Structure seguido por revestimento Diamond-Like-Carbon (DLC) em diferentes espessuras foi possível personalizar o perfil de molhabilidade, alterando a energia de superfície do titânio, e minimizar os efeitos indesejáveis de interação com sangue. Após estudos preliminares, quatro diferentes superfícies foram desenvolvidas: (I) Titânio puro como Material de Referência (MR), (II) Titânio Texturizado como Superfície de Teste (ST), (III) Titânio Texturizado com DLC 0,3 μm de espessura como ST A e (IV) Texturizado com DLC 2,4 μm de espessura como ST B. A caracterização física e química destas superfícies foi realizada utilizando Microscopia Eletrônica de Varredura (Field Emission Gun), Energy Dispersive X-Ray (EDX), Espectroscopia de espalhamento Raman e goniometria. A Avaliação In Vitro da Molhabilidade com Sangue Humano provou a reprodutibilidade e estabilidade dos efeitos observados com água. Para atender a norma ISO 10993, implantes em titânio grau médico foram confeccionados e tiveram suas superfícies biofuncionalizadas; quatro grupos foram considerados, um para cada tipo de superfície e um único implante foi posicionado na aorta abdominal de ratos Wistar. Os efeitos de interação hemodinâmica foram avaliados sem adição de anticoagulantes e depois de doze semanas, os implantes foram extraídos e submetidos à análise qualitativa por MEV em baixo vácuo, 130 Pa, e EDX. As regiões que permaneceram em contato com a parede da aorta abdominal apresentaram encapsulamento por tecido endotelial, formação de pannus. O revestimento DLC inibiu a adesão de material biológico no implante ST B, ainda que de perfil super-hidrofílico. O DLC também demonstrou prevenir o desgaste abrasivo e a delaminação, comparado ao observado nos implantes ST e ST A. Camadas pseudoneoíntima foram identificadas de forma heterogênea e em maior concentração nas Superfícies de Teste. Para concluir, a superfície do titânio foi melhorada com as microestruturas, mas e principalmente, pelo revestimento DLC; o qual se mostrou capaz de minimizar as respostas negativas da interação in vivo; uma vez que não houve óbitos registrados por trombose, acidente vascular ou embolia pulmonar. Novos estudos experimentais considerando diferentes espessuras do DLC podem esclarecer os avanços na minimização de anticoagulantes na assistência ventricular usando bombas de sangue com superfícies biofuncionais. ABSTRACT: Cardiovascular diseases are still the leading cause of mortality worldwide. The high reactivity of blood with any non-endothelial surface is a challenge in the development of long-lasting Ventricular Assist Devices, generally manufactured from medical grade II titanium, which despite being inert, low density and high mechanical resistance does not prevent thrombogenic responses. By modifying the titanium surface with Laser- Induced Periodic Surface Structure followed by Diamond-Like-Carbon (DLC) coating in different thicknesses, it was possible to customize the wettability profile, changing the surface energy of the titanium, and minimizing undesirable effects. interaction with blood. After preliminary studies, four different surfaces were developed: (I) Pure Titanium as Reference Material (MR), (II) Textured Titanium as Test Surface (ST), (III) Textured Titanium with 0.3μm thick DLC as STA and (IV) Textured with 2.4 μm thick DLC as STB. The physical and chemical characterization of these surfaces was carried out using Scanning Electron Microscopy (Field Emission Gun), Energy Dispersive XRay (EDX), Raman scattering spectroscopy and goniometry. The In Vitro Assessment of Wettability with Human Blood proved the reproducibility and stability of the effects observed with water. To comply with the ISO 10993 standard, medical grade titanium implants were manufactured and their surfaces were biofunctionalized; four groups were considered, one for each type of surface and a single implant was positioned in the abdominal aorta of Wistar rats. The hemodynamic interaction effects were evaluated without the addition of anticoagulants and after twelve weeks, the implants were extracted and subjected to qualitative analysis by SEM at low vacuum, 130 Pa, and EDX. The regions that remained in contact with the wall of the abdominal aorta showed encapsulation by endothelial tissue, formation of pannus. The DLC coating inhibited the adhesion of biological material to the STB implant, despite its superhydrophilic profile. DLC has also been shown to prevent abrasive wear and delamination compared to that seen with ST and STA implants. Pseudoneointima layers were identified heterogeneously and in greater concentration on the Test Surfaces. To conclude, the titanium surface was improved with microstructures, but mainly by the DLC coating; which proved capable of minimizing negative responses from the interaction in vivo; since there were no deaths recorded due to thrombosis, stroke or pulmonary embolism. New experimental studies considering different DLC thicknesses may clarify advances in minimizing anticoagulants in ventricular assistance using blood pumps with biofunctional surfaces. |
